Medical Case Management Position – Southwest MN
RAAN is seeking a 1.0 FTE Medical Case Manager to provide services to a diverse population of individuals living with HIV/AIDS in Minnesota.
The position, based in Southwest Minnesota, requires a working knowledge of HIV/AIDS, social services, and other community resources. Applicants must have a valid driver’s license and insurance, and reliable transportation; travel in and around Minnesota is required.
Candidates must have effective communication and problem-solving skills and must be a self-starter, able to manage time and production with off-site supervision.
The ideal candidate will have a Bachelor’s Degree or higher and 3 – 5 years’ experience working in the HIV/AIDS field, social services, public health, medical field, or other related field; bilingual skills a plus.

The Rural AIDS Action Network (RAAN) assists persons living with, affected by and at risk for HIV/AIDS in rural Minnesota. RAAN offers medical case management, HIV/HCV counseling, testing and referral, harm reduction services including naloxone training and distribution and syringe services as well as education and training services statewide.
