Partner Clinics
RAAN works with different health systems to provide high quality HIV care across rural Minnesota. This is accomplished by creating partnerships designed to utilize the expertise of each agency to provide HIV Health Services, Comprehensive HIV/AIDS Education and Medical Case Management Services to people living with HIV/AIDS. This partnership is mutually beneficial to both agencies and their patients/clients. RAAN’s expertise is in providing medical case management, HIV testing and care coordination for rural persons living with HIV/AIDS; and the clinics expertise is in providing Primary Medical Care and/or Infectious Disease/ HIV Care. Through this partnership, the agencies are able to provide the following services in a way neither can provide alone:
- Primary Medical care and Infectious Disease/ HIV Care
- Health care provider education/training; and
- Provide HIV/AIDS health education;
- Disease prevention and control;
- Expert rural HIV Medical Case Management Services.
- Free and confidential HIV testing
